Incoming Senior Immunization Requirement Additional Information
A newer requirement for the State of Ohio for school attendance is the addition of the Mennigitis vaccine for incoming seniors. (See below for specific wording). The overall requirement is: your son needs at least one Mennigitis shot (MCV4) on or after his 16th birthday. Last year the Pediatricians, Little Clinics, and Walgreens ran out at the beginning of the school year due to poor planning. Please schedule this appointment now. Please send a copy of your son's shot records to school once he receives the vaccine so we can document for the State his immunization. I MUST have a copy of the shot record due to reporting purposes to the State. Any questions please contact me at email@example.com or 741-2302.
"Recommended MCV4 minimum interval of at least eight (8) weeks between dose one (1) and dose two (2). If the first (1st) dose of MCV4 was administered on or after the 16 th birthday, a second (2nd) dose is not required. If a pupil is in 12th grade and is 15 years of age or younger, only 1 dose is required. Currently there are no school entry requirements for meningococcal B vaccine."
