Sport Participation Requirements and Forms
It is a requirement of the Ohio High School Athletic Association that a student athlete wishing to participate in interscholastic sports must have a yearly physical, and all required medical forms must be on file in his school’s Athletic Department prior to the start of the sport season.
Academic Athletic Eligibility is based on a student athlete’s grades from the previous quarter. In order for an Incoming Freshman to be academically eligible to participate in a Fall Sport, the Ohio High School Athletic Association requires that he pass 5 classes in the 4th Quarter of his 8th grade year. Summer school grades do not count. A copy of his 4th Quarter Report Card must be turned in to the Athletic Department along with his medical forms.
The following is a list of 2014-15 sports offered at La Salle with the deadline date that all medical forms need to be turned into the Athletic Department.
Fall Sports: Cross Country, Football, Golf, Soccer
Winter Sports: Basketball, Bowling, Ice Hockey, Swimming & Diving, Wrestling
Spring Sports: Baseball, Lacrosse, Tennis, Track & Field, Volleyball

OHSAA Preparticipation Form and OHSAA Authorization Form
OHSAA has announced a partnership with PrivIT, a Columbus based company, to streamline the process for the management and collection of physical forms required for participation in high school sports.
The OHSAA will be replacing the paper physical forms with the PrivIT e-PPE (electronic Pre-Participation Evaluation( secure online solution beginning May 1 for the 2014-15 academic year.

are to be completed and signed by the parent and student athlete.
Beacon Authorization to Disclose Personal Health Information
This form must
be returned to the Athletic Department even if the physical was not completed by Beacon Orthopaedics & Sports Medicine.

Form is to be read by both athlete and parent. Sign the last page and return it to the Athletic Office
